*Dental Insurance Employee Paid
Altus Dental and we offer two different dental plans a high plan with coverage of up to $1500 per year and a low plan offer basic dental benefits.
*SHORT TERM DISABILITY Company Paid
Provides a benefit of 60% of salary once an employee is out of work due to a non work related disability or illness for 14 days for up to 13 weeks.
*LONG TERM DISABILITY Employee and Company Paid
Provides a benefit of 60% of salary up to $4,000 per month once an employee exhausts STD benefits. Benefit continues until employee is no longer disabled or reaches social security normal retirement age or age 65.
*GROUP LIFE INSURANCE Company Paid
Provides a life insurance benefit of $10,000 to beneficiary designated by employee.
Provides an accidental death and dismemberment benefit of $10,000 to employee or beneficiary designated by employee.
*SUPPLEMENTAL LIFE INSURANCE Employee Paid
Employee may elect from 1X – 5X of their annual salary up to $500,000
($75,000 guaranteed issue)
Dependent Life Insurance Employee Paid
Participation in this benefit allows employees already enrolled in Supplemental Life Insurance to elect various amounts of life insurance coverage for their spouses and/or dependent children
*TUITION ASSISTANCE Company Paid
Employees may apply for tuition assistance benefits upon completing six months of service. Maximum benefit $1,500 per semester per employee.
Employees may also apply for Tuition Remission Benefits
403(b) PLAN
Opportunity to participate in a tax deferred retirement investment plan through payroll deduction. Employees may invest a percentage of their salary or a specific amount up to federally allowable contributions with pre-tax dollars.

PAID TIME OFF BENEFITS
There are specific waiting periods before employees are eligible to participate in our benefit plans.
Paid time off benefits are based on service requirements, employee status (hourly or salaried), and bargaining unit status. Paid time off benefits include sick time, personal time, holiday pay, and vacation time.
An employee must be scheduled to work a minimum of 30 hours per week in a “regular” position to be eligible to participate in all benefit plans except medical insurance.
An employee must be scheduled to work a minimum of 35 hours per week in a “regular” position to be eligible to participate in our medical insurance plan.
All eligible employees may enroll in the medical insurance plan upon completing 90 days of service with the agency.
***Eligibility for participation in benefit plans depends on employee status hourly or salaried and bargaining unit or non bargaining unit status.
Hourly and salaried employees regularly scheduled to work at least 35 hours per week are eligible to participate in the medical insurance plan
upon completing 90 days of service.
For all other benefits hourly employees are eligible at 6 months.
There are specific waiting periods before employees are eligible to participate in our benefit plans and there is a minimum number of hours of work required before employees are considered benefit eligible.
